The paramount aim of the service is to provide facilitated follow-up that helps persons master their living conditions and can live safely and well in their own home. The service shall provide uniform follow-up that promotes the users' experience of self-mastering,

safety and good quality of life in their own home. The service shall include follow-up in all areas that are significant for the users ́ living conditions and that promote stability, mastery and self-care. The service shall be provided to
